Welcome to the

13^th annual Leah Fenney Memorial Soccer Tournament

to be held on Sunday, September 16^th, 2007.

Attached is a waiver to be signed by a parent or guardian.

There is a play area for kids, including playground equipment, basketball and volleyball courts, so bring the whole family!

Food and drink will be available at the park.  No carry-ins please!

Please sign and return the following release form ASAP to your child’s coach so that your child may participate in the Leah Fenney Memorial Soccer Tournament.
Players will not be allowed to play without a signed release form.

Liability Waiver:

I hereby hold the Erin Soccer Club free and harmless from any liabilities which may arise while my child is participating in the Leah Fenney Memorial Tournament.  I acknowledge I must have adequate medical insurance to cover any injury that may occur while participating in this tournament.
