Leah Fenney Memorial Soccer Tournament
to be held on
Sunday, September 16th, 2007.
Attached is a waiver to be signed by a parent or guardian, as well as a volunteer sign up form.
The majority of proceeds from this event will go to the Erin Soccer Club, which will directly impact your soccer fees in upcoming years. Therefore, we are asking anyone who is not volunteering in some capacity for your club to step up and uphold your commitment to volunteer that you made on your child’s soccer registration form. We cannot have a successful event without the help of all club members.
Food and drink will be available at the park. No carry-ins please!
Please sign and return the following two forms ASAP to your child’s coach so that your child may participate in the Leah Fenney Memorial Soccer Tournament. Players will not be allowed to play without a signed release form.
Liability Waiver:
I hereby hold the Erin Soccer Club free and harmless from any liabilities which may arise while my child is participating in the Leah Fenney Memorial Tournament. I acknowledge I must have adequate medical insurance to cover any injury that may occur while participating in this tournament.
